- A statement that the information in the notice is accurate, and, under penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.
Title 17 USC §512(f) provides civil penalties, including costs and attorney’s fees, for any person who knowingly misrepresents certain information in an infringement claim under 17 USC §512(c)(3).

Counter Notification – Restoration of Material
If you have received a notice regarding the takedown of material due to a copyright infringement claim, you can submit a counter notification to have the material restored. Your counter notification must include:
- Your physical or electronic signature.
- A description of the material that was taken down and the original location of the material.
- A statement under penalty of perjury that you have a good faith belief that the material was removed as a result of mistake or misidentification.
- Your name, address, telephone number, and a statement consenting to the jurisdiction of the federal district court in your district (or if outside the U.S., any judicial district where the service provider may be found), and that you will accept service of process from the person who filed the original notice.

Repeat Infringer Policy
